The rest of its body looks fine.

Looks like your local orandas are in a pretty bad shape. This is totally up to you to save him as to me just by a glance....it has a minor case of fin rot & a weak stomach. With this signs together with lethargic behaviour this fish is pretty hard to cure. It looks real weak, so if you introduce meds....it might be too caustic for it to even survive medicated environment.
But there is something you can try though. If you wanna giveit a last shot, try putting him under 0.5% salinity, WC(water change) every 2 days. If you dun understand the salinity measurement....here it goes:
0.1% salinity = 1gram per liter
0.3% salinity = 3grams per liter
0.5% salinity = 5grams per liter
Give this guy minimal feeding not to deteriorate the water condition....I hope he's still eating. All the best!
